
To start a foreclosure,
please follow these steps:
1. Download the appropriate PDF documents below. These documents utilize interactive PDF functions and can be filled out and saved digitally. Alternatively, you may print them out and fill them manually. You must have the free Adobe Reader (or Adobe Acrobat) installed to view and fill these forms. If you do not have Adobe Reader, you may download, for free, the application from here or by visiting the site link shown.
Important: If completing digitally, please complete the form and then Save and/or Print form before closing, or your information will be lost.
Foreclosure Information Form: Use this form if default occurred under a Deed of Trust.

HOA Foreclosure Information Form: Use this form if default occurred under a homeowners association (HOA) Delinquent Assessment Lien
2. Gather the support documents requested on the Information Form. You may submit them in the same manner that you are submitting the Information form: via Email, Fax, or Mail Service.
Note: Clear copies of the original documents are acceptable to start the foreclosure; however we will require you to provide the original documents before the Trustee's sale.

3. Upon receipt of the completed Information Form and pertinent support documents, we will start the foreclosure, and then send you a Substitution of Trustee and Declaration of Default & Demand For Sale for your signature and notarization. Please return the original signed and notarized documents to Pacific Oak Foreclosure Services, Inc. via a traceable service, such as overnight or certified first class mail; copies sent via Email or Fax are not acceptable.
4. A $500.00 deposit is required to start the foreclosure.
